Foreign subpoena filing in Utah

Speaking to the court clerk at the Salt Lake District Court she advised, that although the whole Utah court system is under red alert, it's still necessary to enter the courthouse to file subpoenas that must have an original signature for out of state filings of foreign Subpoenas
. 
Utah Courts COVID-19 ALERTS

To file/conform a foreign subpoena the Utah court needs:

• Civil Filing Cover Sheet.
• $35 court filing fee.
• Utah Application for Subpoena - prepared with original signature. or
• Notice of Deposition and Request for Subpoena in Case Pending Out of State - Use this form if your state has not enacted the UIDDA.
  
• Subpoena(s) - prepared with original signature.
• Foreign subpoena with any associated documents.
  

When we serve the filed documents we'll include in each subpoena package:

• Filed Application with a Utah case number (or UIDDA form).
• Filed Subpoena(s) with Utah case number and court stamps.
• Utah Notice to Persons Served with a Subpoena.
• Utah Objection to Subpoena. (included with subpoenas for appearance)
• Utah Declaration of Compliance with a Subpoena. (included with subpoenas for document production [Subpoena Duces Tecum])
• Foreign subpoena with any associated documents. 

Only one application (or UIDDA form) is needed to get a single case number for more than 1 subpoena. Provide a separate subpoena for each person/entity you intend to serve.

Check Advance
We can advance the court filing fee of $35.00
Generally, Utah subpoenas for appearance include a check to the subject for $18.50
We can advance the "witness fee".
